Responsibilities- Provide direct patient care to residents, including administering medications, wound care, and vital sign checks.
- Educate patients and families about disease treatment plan, including self-care post discharge.
- Monitor and report changes in resident condition to the nursing team and attending physicians.
- Assist with resident transfers, ambulation, and mobility exercises.
- Assure resident safety by identifying safety hazards, emergency situations and initiating corrective action as appropriate.
- Maintain accurate and up-to-date records of resident care and treatment.
- Participate in quality improvement initiatives to promote excellent patient outcomes.
- Collaborate with other departments to ensure seamless delivery of care.
- Currently licensed as a Licensed Practical Nurse (LPN) in the State of New York.
- Must be able to speak, read and follow instructions in English.
- BLS Provider obtained within 3 Months (90 days) of hire date or Job transfer date.
- Ability to work variable shifts, including evenings and weekends.
- Strong communication and interpersonal skills.
- Basic computer skills and familiarity with electronic medical records.
