Licensed Practical Nurse (LPN) Evenings

Job Summary:

Wilkinson Residential Health Care Facility is a leading healthcare company, seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in Amsterdam, NY. Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital sign checks.

• Educate patients and families about disease treatment plan, including self-care post discharge.

• Monitor and report changes in resident condition to the nursing team and attending physicians.

• Assist with resident transfers, ambulation, and mobility exercises.

• Assures resident safety by identifying safety hazards, emergency situations and initiating corrective action as appropriate.

• Maintain accurate and up-to-date records of resident care and treatment.

• Participate in quality improvement initiatives to promote excellent patient outcomes.

• Collaborate with other departments to ensure seamless delivery of care.

Requirements:

• Currently licensed as a Licensed Practical Nurse (LPN) in the State of New York.

• Must be able to speak, read and follow instructions in English.

• BLS Provider obtained with within 3 Months (90 days) of hire date or Job transfer date.

• Ability to work variable shifts, including evenings and weekends.

• Strong communication and interpersonal skills.

• Basic computer skills and familiarity with electronic medical records.

• Compassionate and caring attitude towards patients and their families.
