• Provide direct patient care to residents, including administrations of medications, wound care, and vital sign checks
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report changes in patient conditions to the nursing staff and physicians
• Maintain accurate and comprehensive records of patient care
• Participate in quality improvement initiatives to enhance patient outcomes and facility operations

• Current LPN licensure in the state of New York
• At least 1 year of experience in a long-term care setting
• Certificate in Practical Nursing
• Ability to work a full-time schedule
