Nurse Practitioner Or Physician Assistant
Community Care Physicians (CCP) is seeking an experienced or highly motivated Nurse Practitioner or Physician Assistant to join Vascular Health Partners, one of the Capital Region's premier vascular surgery practices.
This is an outstanding opportunity to work alongside four board-certified vascular surgeons and an experienced team of advanced practice providers specializing in the diagnosis and treatment of complex arterial and venous disease. Vascular Health Partners is nationally recognized for its expertise in minimally invasive endovascular procedures, vascular surgery, research, and patient education.
Providers practice primarily from our state-of-the-art office and outpatient interventional suite located at Wellness Way in Latham, with scheduled travel to our satellite offices in Saratoga Springs, Queensbury, and Amsterdam to provide outpatient vascular care.
Practice highlights include:
- Four 10-hour workdays (4-10 schedule)
- 32 hours of scheduled patient care and 8 hours of dedicated administrative time each week
- Outpatient-focused practice with no overnight hospital rounding requirements
- State-of-the-art outpatient vascular interventional suite in Latham
- Accredited on-site noninvasive vascular laboratory
- Collaborative team of four vascular surgeons and experienced APPs
- Opportunity to practice cutting-edge vascular medicine utilizing the latest minimally invasive technologies
- Practice locations throughout New York's Capital Region, including:
- Latham (primary location)
- Saratoga Springs
- Queensbury
- Amsterdam
Responsibilities include:
- Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting
- Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons
- Manage patients with conditions including:
- Peripheral arterial disease (PAD)
- Carotid artery disease
- Abdominal and peripheral aneurysms
- Chronic venous insufficiency
- Varicose veins
- Deep vein thrombosis (DVT)
- Peripheral vascular disease
- Provide pre-operative and post-operative evaluations and follow-up care
- Review vascular ultrasound and diagnostic testing results
- Coordinate care with referring primary care physicians and specialists
- Educate patients regarding vascular disease prevention, treatment options, and lifestyle modifications
Qualifications:
Required:
- Current New York State licensure as a Nurse Practitioner or Physician Assistant
- Graduate of an accredited NP or PA program
- DEA registration (or ability to obtain)
- Strong clinical assessment and communication skills
- Ability to work independently while collaborating closely with physicians
Preferred:
- Previous experience in vascular surgery, surgery, cardiology, interventional radiology, emergency medicine, or critical care
- New graduates with prior surgical or vascular clinical rotations and a strong interest in vascular medicine are encouraged to apply
