• Assess patients' functional abilities and develop individualized occupational therapy treatment plans
• Implement evidence-based interventions for home health and outpatient populations
• Document thoroughly and accurately in the Electronic Medical Record to support continuity of care
• Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes
• Work with physician referrals to ensure treatments align with O.T. practice guidelines
• Participate in quality and safety initiatives to continuously improve patient care
• Educate patients and caregivers to promote independence, safety, and meaningful daily activity

• OTR credential (Registered Occupational Therapist) — this role is for an OTR as stated
• Experience in home health and/or outpatient settings preferred (demonstrated patient care experience)
• Proficiency with Electronic Medical Record systems and strong documentation skills
• Ability to work collaboratively with interdisciplinary teams and follow AOTA practice guidelines
• Authorized to treat with physician referral (per job requirements)

• Strong clinical knowledge of occupational therapy principles and interventions
• Skilled in patient assessment, treatment planning, and outcome measurement
• Comfortable working in home health and outpatient environments
• Excellent documentation and EMR navigation skills
• Ability to collaborate with physicians, nurses, and therapy colleagues
• Familiarity with quality and safety practices in patient care
