Position: Charge Nurse LPN (12 Hr Shifts)

We are currently seeking a Licensed Practical Nurse (LPN) to join our Wellness team.

The Licensed Practical Nurse (LPN) provides direct nursing care to assisted living residents, ensuring safe, compassionate, and person-centered care. This role works closely with the Wellness Director and interdisciplinary team to support residents’ health needs and overall well‑being.

The LPN plays a key role in monitoring residents, communicating changes in condition, and ensuring care plans are followed accurately in accordance with state regulations and community standards.

  • Status: Full-Time or Part-Time
  • Shifts: 12-hour shifts (8:00 AM – 8:00 PM)
  • Department: Wellness / Nursing
  • Administer medications and treatments in accordance with state regulations and physician orders
  • Monitor residents’ conditions and promptly report changes to the appropriate clinical leadership
  • Provide direct nursing care, including wound care, vitals, and basic clinical interventions
  • Support residents with activities of daily living (ADLs) as needed
  • Maintain accurate, timely, and compliant documentation in the medical record
  • Collaborate with caregivers, providers, and families to ensure continuity of care
  • Follow infection control protocols and safety standards
  • Ensure compliance with community policies, procedures, and quality standards
  • Participate in care planning and interdisciplinary team communication
Requirements
  • Graduate of an accredited nursing program
  • Active LPN license in the state of employment
  • Current CPR certification
  • Senior care or healthcare experience preferred
  • Ability to work independently and as part of a team
