We are currently seeking a Licensed Practical Nurse (LPN) to join our Wellness team.
The Licensed Practical Nurse (LPN) provides direct nursing care to assisted living residents, ensuring safe, compassionate, and person-centered care. This role works closely with the Wellness Director and interdisciplinary team to support residents’ health needs and overall well‑being.
The LPN plays a key role in monitoring residents, communicating changes in condition, and ensuring care plans are followed accurately in accordance with state regulations and community standards.
- Status: Full-Time or Part-Time
- Shifts: 12-hour shifts (8:00 AM – 8:00 PM)
- Department: Wellness / Nursing
- Administer medications and treatments in accordance with state regulations and physician orders
- Monitor residents’ conditions and promptly report changes to the appropriate clinical leadership
- Provide direct nursing care, including wound care, vitals, and basic clinical interventions
- Support residents with activities of daily living (ADLs) as needed
- Maintain accurate, timely, and compliant documentation in the medical record
- Collaborate with caregivers, providers, and families to ensure continuity of care
- Follow infection control protocols and safety standards
- Ensure compliance with community policies, procedures, and quality standards
- Participate in care planning and interdisciplinary team communication
- Graduate of an accredited nursing program
- Active LPN license in the state of employment
- Current CPR certification
- Senior care or healthcare experience preferred
- Ability to work independently and as part of a team
