The Registered Nurse Navigator
- Oncology Clinic provides care coordination, patient education, and resource navigation for patients receiving evaluations, diagnosis, surgical treatment, or ongoing management of oncologic conditions within Island Health specialty and surgical services. The navigator serves as a consistent point of contact for patients and families, helping reduce barriers to timely, high-quality, patient-centered cancer care through coordination with internal and external providers.
The Oncology Nurse Navigator collaborates with physicians, advanced practice providers, nursing staff, ancillary departments, social services, community agencies, and regional oncology partners to facilitate seamless transitions across the continuum of care. The position also assists patients with insurance navigation, financial assistance resources, and access to supportive services to help minimize barriers to treatment.
What you will be doing:
- Conducts initial screening and ongoing assessment of patients and families to identify psychosocial, financial, and practical barriers to care.
- Leads and maintains accountability for the oncology nurse navigation program, including development, implementation, evaluation, continuous improvement, and monitoring of program metrics and outcomes.
- Develops, implements, and maintains standardized oncology navigation processes, workflows, and resources in collaboration with clinical leadership.
- Serves as an organizational subject matter expert and resource for oncology navigation; offers clinical guidance, education, and mentorship to nursing and interdisciplinary staff.
- Leads applicable oncology quality, regulatory, and accreditation activities and represents Island Health with regional oncology partners and other external stakeholders.
- Assess patients for symptom management needs during the course of treatment and communicates with the care team regarding identified needs.
- Utilizes and maintains standardized psychosocial assessment tools to support care coordination.
- Educates patients on treatment plans, side effect management and harm avoidance.
- Provides information and referral services; develops and maintains resource lists for medical equipment, social services, transportation assistance, lodging, support services, and other patient resources.
- Collaborates with pharmacy, financial counseling, billing staff, infusion services, specialty providers, and external oncology partners to facilitate timely access to prescribed therapies and supportive care.
- Works with local and national service organizations, including the Island Hospital Foundation, to coordinate grant funding and patient assistance resources.
- Assists uninsured or underinsured patients in identifying and accessing appropriate financial and community support programs.
- Completes agency-specific referral forms and required documentation in accordance with regulatory and program requirements.
