About The Role
Clarvida is seeking a full‑time Care Manager who will deliver outreach, information‑referral, care management, and coordination of mental health/rehabilitation services. The Care Manager will work directly with clients to assess needs, develop care plans, and collaborate with families and providers within Clarvida’s trauma‑informed, person‑centered care model.

- Conduct intake and comprehensive evaluations of clients’ life circumstances, challenges, and goals.
- Prepare detailed care plans with client participation and consent.
- Link clients to community resources and support services, and monitor effectiveness.
- Maintain regular contact, including home visits, providing support, guidance, consultation, and intervention as needed.
- Maintain daily records of service progress, billing forms, and ensure compliance with company and government standards.
- Conduct outreach and information/referral for individuals who do not require full case management services.
- Uphold HIPAA, ethical standards, and professional boundaries with all staff and clients.
- Provide temporary intervention to ex‑clients when needed.
- Develop and sustain relationships with external agencies and providers.
- Participate in staff meetings, training, and organizational activities.
- Meet monthly productivity requirements and documentation standards.
- Transport clients when required; personal vehicle required for travel.
- Handle on‑call or after‑hours duties as assigned.
- B.A./B.S. degree in Human Services, Psychology, or related field.
- Minimum 2 years of case management or equivalent behavioral health experience.
- Strong interpersonal skills and excellent organizational, communication, and prioritization abilities.
- Valid California driver’s license, auto insurance, and reliable transportation.
- Proof of education, current CPR certification required.
