Licensed Vocational Nurse

La Casa Health Care in Belen, NM seeks a Licensed Vocational Nurse to provide compassionate, resident-centered nursing care in a skilled nursing and long-term care setting. The LVN will deliver direct patient care, administer medications and treatments, monitor residents' conditions, and promptly report changes to the RN or physician. Responsibilities include wound care, documentation, assisting with rehabilitation plans, and collaborating with an interdisciplinary team.

The LVN will educate residents and families, support care plans that respect dignity and preferences, and help maintain a safe, supportive, and community-focused environment for seniors.

Responsibilities

• Provide direct nursing care to residents in skilled nursing and long-term care settings
• Administer medications and treatments per provider orders and facility protocols
• Monitor, document, and report changes in residents' conditions to RN or physician
• Perform wound care and assist with rehabilitation and restorative care plans
• Accurately complete electronic health records and required clinical documentation
• Collaborate with interdisciplinary team members to develop and follow care plans
• Educate residents and families on care, medications, and health management
• Support a safe, dignified, and resident-centered environment for seniors
• Assist with admissions, discharges, and transitions of care
• Follow infection control, safety, and regulatory standards at all times
