Job Summary
This position involves extensive coordination, planning, and evaluating patients and community members with diagnosed heart failure or symptoms of heart failure. The Cardiac Rehab Coordinator will provide rounding on patients hospitalized with diagnoses of CHF or potential for CHF or signs and symptoms thereof. The Coordinator will gather information to ensure that the core measures for quality for this patient diagnosis are achieved and coordinate education for staff caring for patients with heart failure and provide community education lectures.
The Coordinator will communicate effectively with physicians involved in the care of these patients, chair the Heart Failure multidisciplinary team, and plan and provide cardiac and pulmonary rehabilitative care while assuring patient safety. The Coordinator will conduct individual needs assessments, plan, implement, and monitor the patient’s exercise prescription and risk‑factor reduction plan. This care aims to improve physical function, general well-being, and prognosis while promoting independent self‑management through education.

- Coordinate education for staff and provide community education lectures.
- Communicate effectively with physicians and chair the Heart Failure multidisciplinary team.
- Plan and provide cardiac and pulmonary rehabilitative care, ensuring patient safety.
- Conduct individual needs assessments and establish exercise prescriptions and risk‑factor reduction plans.
- Support Anaheim Regional Medical Center’s strategic plan, goals, and performance improvement plan.
- Support organizational expectations including Customer Service, Patient's Rights, Confidentiality of Information, Environment of Care, and Memorial Care initiatives.
- RN from an accredited nursing program (preferred).
- Three years of clinical health care or health education experience (preferred).
- Effective oral and written communication skills.
- Ability to provide individual and large group counseling sessions.
- Experience in cardiology nursing.
- Experience with data collection.
- BLS certification.
Current CA RN license preferred.
