Home Health Registered Nurse Case Manager

Accent Care is seeking a Registered Nurse / Patient Care Manager Regional Floater for our home health team. In this role, you will travel across a defined region to deliver skilled nursing care, manage complex patient caseloads, and support local teams. You will assess patients, create and update care plans, coordinate with physicians and pharmacists, and ensure compliance with home health regulations.

Working within a collaborative, innovation-focused culture, you will help drive high-quality, patient-centered care and mentor staff while gaining exposure to advanced pharmaceutical and clinical practices.

Responsibilities

• Provide skilled nursing care to home health patients across assigned region
• Float between locations to support patient care coverage and case management
• Develop, implement, and update individualized patient care plans
• Coordinate with physicians, pharmacists, and interdisciplinary teams on treatment plans
• Monitor patient progress, document care, and ensure regulatory compliance
• Educate patients and families on medications, therapies, and home care procedures
• Uphold infection control, safety, and quality standards in all settings
• Mentor and support clinical staff to ensure consistent best practices
