LHH seeks a dedicated Case Manager to join its Interim Housing team in Anaheim, CA. You will support participants through street outreach, interim housing, and permanent housing placements, applying a Housing First and harm reduction approach.
You will conduct intakes, develop ISP plans addressing housing, income, and health needs, and connect clients with community resources while maintaining accurate documentation.
