Community Services Coordinator- Project SAY

The City of Anaheim Community Services Department seeks a highly dynamic Community Services Coordinator to support the City's youth development efforts. The Community Services Coordinator will research, develop, and conduct youth development and outreach services to junior high and high school age youth and their families living in "at risk" environments.

Project S.A.Y. (Support Anaheim's Youth) is the City of Anaheim's Youth Development Program. Project S.A.Y. serves Junior High and High School age youth and their families living in Anaheim and uses a combination of Youth Outreach, Case Management, School Interventions, Assistance to Parents, Community Education, Youth Leadership Development, Health and Fitness, and Education and Employment to engage youth in positive and constructive activities to promote the healthy development of young people.

Youth meet individually or in small groups with a Community Services Coordinator to resolve conflict and problem solve issues of importance in a young person's life. Youth participate in a wide range of social, recreational, and educational activities that help them develop the knowledge, skills, and abilities necessary for successful adulthood. Participants develop leadership, conflict resolution, problem solving, and goal-setting skills.

Youth help plan their own activities such as sporting events, camping trips, leadership development programs, and service projects in the community. Project S.A.Y. services are offered at no cost to Junior High and High school age youth.

Bilingual proficiency (read/speak/write) in Spanish is highly desirable.

This position requires a flexible work schedule which includes some evening and weekend shifts.

• Develop, coordinate and implement youth development programs/activities that help youth build skills in the five competency areas identified as critical to the healthy development of young people (social/emotional, health/physical, cognitive/creative, vocational, and citizenship/ethical).
• Conduct behavior guidance and para-professional counseling with youth.
• Provide case management services including assessing client needs, coordinating a service delivery plan, providing para-professional counseling, and conducting follow-up checks on success of client/agency linkage, and conduct client progress checks.
• Monitor and report client progress toward independence; establish and prepare case management files; prepare and maintain long term tracking records of clients; prepare and maintain statistical and narrative reports, productivity records, daily staff logs, project plans, and incident reports.
• Conduct school based gang, drug, and violence prevention/intervention services, and assist school administrators in tracking youth living in "at-risk" environments and assisting them with reentry into school.
• Conduct in-home assessments of family situation/needs and development of service delivery plan; may include conferences with parents regarding status of child welfare; establish individual responsibility, behavior, and academic agreements with clients; conduct emergency assistance and provide crisis intervention and follow-up to families; conduct initial assessment of youth, redirect youth, and follow up as needed on all referrals from school administrators, police officers, probation officers and parents.
• Provide conflict resolution and mediation assistance between youth and their peer, parents, schools, and community.
• Provide in-depth information and referral services; identify on-going support services/needs of families and assist them in accessing those services; identify short-term and long-term educational needs of clients and provide responsible referrals to social service agencies for assistance; problem solve obstacles to client progress; instruct clients on how to self-assess and access services.
• Develop opportunities for youth that help them build developmental assets that youth need to grow up caring and responsible. (Asset categories:
  Support, Empowerment, Boundaries & Expectations, Constructive Use of Time, Commitment to Learning, Positive Values, Social Competencies, Positive Identity) Guide and supervise youths in the asset categories.
• Participate in planning meetings and implementation of special events, educational programs and health services; conduct parent education presentations on subjects such as gang/drug awareness and prevention, graffiti prevention, behavior modification and communication; provide in-service training to school personnel and other youth service professionals; provide bilingual services including translating and transcribing English to Spanish and Spanish to English, and completion of public assistance forms.
• Maintain effective communication and working relationships with patrons, participants, clients, co-workers, City Department representatives, and outside agencies.
