Polaris Transitional and Extended Care in Anchorage, AK, is seeking a dedicated Full Time Occupational Therapist ready to make a meaningful impact. The extended care facility is inspired by the Green House care model, offering top‑tier skilled nursing and rehabilitation services within a homely ambiance for its residents. 96 beds are spread over eight free‑standing cottages, each with a private room and a central open‑plan kitchen and dining area that promotes social interaction. The transitional care facility is a 50‑bed, high‑tech environment with individual rooms and a therapy gym equipped with high‑tech equipment and ample space for a passionate therapist.

Key Responsibilities- Effectively screens and evaluates patients with cognitive and/or ADL functional disorders and develops an appropriate plan of care following all regulatory and clinical practice standards.
- Utilizes standardized assessments and evidence‑based practice to support clinical interventions.
- Provides rehabilitative, skilled, and medically necessary treatment interventions for patients with cognitive and/or ADL functional disorders per regulatory and clinical practice requirements.
- Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluates treatment outcomes for effectiveness; services are modified in relation to patient evaluation and referrals are made as appropriate.
- Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provides comprehensive discharge summaries per regulatory and clinical requirements.
