Overview
The Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive patient-centric care management services for at-risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access.
We are hiring for Social Worker MSW positions at Providence Alaska Medical Center in Anchorage, AK.
Responsibilities- Provide professional, comprehensive patient-centric care management services in an acute care environment.
- Perform assessment and planning, coordinate care, discharge planning, document interventions, ensure regulatory compliance, and advocate for patients.
- Identify resources to address social, financial, cognitive/behavioral or legal barriers to care access and promote continuity of care.
- Collaborate with care teams to support at-risk patients and their families.
- Full time, 1.0 FTE, Day shift, 40 hours per week.
- Full time, 1.0 FTE, Night shift, 40 hours per week.
- $10,000 Hiring Bonus for eligible external hires that meet required qualifications and conditions for payment.
- Relocation Assistance available for eligible hires that meet required qualifications and conditions for payment.
- Wage range: $35.32 - $54.82 per hour. The base pay range may be supplemented by differential or bonuses.

- Master s Degree in Social Work from a CSWE-accredited program.
- Alaska Master Social Worker License or Alaska Master Social Worker Temporary License or National Military Courtesy License upon hire.
- 1 year of social work experience in any healthcare setting.
- 1 year of experience in an acute care setting or successful completion of TIPs program or Case Management Orientation Program.
- Basic understanding of acute medical-surgical terminology; prior case management experience.
