Job Summary

The Case Manager RN is responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and other team members are critical to this role.

• Active AK RN License in hand for Auto Offer
• BLS certification
• Clinical Assessment, Medication Administration, and Treatment Planning skills
• Interdisciplinary Team Collaboration skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Bachelor's degree in Nursing or related field
• Driver's license, car insurance, and automobile for field work
• Minimum 1 year of experience

• ACM Certification
• Epic experience
• Acute Care experience

• Weekend requirement:
  Every other weekend or 4 weekend days in a 4-week schedule.
• Clinician will float within scope to meet facility needs.
• Only 1 time off request per 13-week assignment of 5 days or less.
• No holiday RTO for Auto Offer.
• Candidates permanent address must be at least 50 miles from facility.
