Compensation: $19-$22/hour

Schedule: Monday-Friday

Location:Anchorage, AK

As a Bridge Care Coordinator
, you will be an essential part of our team, providing support and companionship to aging patients through our Bridge Program
. Your responsibilities will include:

Visiting patients in their homes and providing peace of mind.

Educating families about available healthcare services.

Connecting patients with community resources.

Helping transition patients between care services, including hospice.

18+ years old

Healthcare experience required

Strong organizational & computer skills

Ability to work independently

Valid driver's license & clean driving record

Reliable transportation & current auto insurance (MVR check required)

Negative TB test
