Carenest Health Services is seeking a travel nurse RN Acute Care Case Management for a travel nursing job in Anchorage, Alaska.

JOB DESCRIPTION:

Position - RN-Case Manager
Location - 3200 Providence Dr, Anchorage, AK 99508

JOB DETAILS :
Full Time - 13 weeks contract
Day Shift - 8 am to 4:30 am (8X5 shift)// 40 hours weekly

Weekend REQ:
Yes - every other weekend or 4 weekend days in a 4 week schedule
Will clinician float within scope to meet facility needs?:
Yes

Ability to commute/relocate:
Anchorage, AK 99508:
Reliably commute or planning to relocate before starting work (Preferred).

RESPONSIBILITIES:
Conduct comprehensive patient assessments to determine medical necessity and discharge needs
Develop and implement individualized discharge plans in collaboration with physicians and interdisciplinary teams
Coordinate post-acute care services including home health, SNF, rehab, DME, and community resources
Perform utilization review to ensure appropriate level of care and prevent avoidable delays
Communicate with insurance providers regarding authorizations and coverage requirements
Facilitate timely discharge planning to reduce length of stay and prevent readmissions
Advocate for patients and families to ensure safe, cost-effective care transitions
Maintain accurate documentation in EMR systems and ensure compliance with hospital and regulatory standards
Participate in multidisciplinary rounds and care coordination meetings
Educate patients and families on discharge instructions and available resources

REQUIREMENT:

At least 2 years of experience within the specialty required
AK RN License
BLS

ACM Cert Preferred
