Summary

The LPN works collaboratively in an interdisciplinary coordinated care delivery model. Primarily responsible for patient and clinic flow, clinic efficiency, customer service, and teamwork. The announcement is open continuously. Applicants will be accepted on an ongoing basis and qualified candidates will be considered. The close date for this announcement is May 18 2026.

Qualifications
  • United States Citizenship:
    Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency:
    Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English.
  • Licensure:
    Must possess a full‑active‑current‑and‑unrestricted licensure as a licensed practical or vocational nurse in a State‑Territory, Commonwealth, or District of Columbia.
  • Education:

    Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission at the time the program was completed.
  • Transcript verification may be obtained from the State approving agency or the National League for Nursing Accrediting Commission.
  • TRANSCRIPTS WITH CONFER DATE REQUIRED.
  • Exceptions include military health education or training accepted by the licensing body as qualifying for full LPN/LVN licensure.
  • Grade Determinations:
    In addition to the basic requirements, the following qualification criteria must be met for each grade: the candidate’s qualifications must clearly demonstrate the level of competence required for the grade.
  • GS‑03 Level: None beyond the basic requirements; LPNs at this grade perform all duties within the scope of their license (e.g., medication administration).
  • GS‑04 Level: At least six months of qualifying experience as an LPN or LVN, or graduate of an approved school plus one year of experience in a hospital, outpatient clinic, nursing home or similar setting.
  • GS‑05 Level: At least one year of qualifying experience equivalent to the GS‑04 level under the general supervision of an RN or MD/DO; broad working knowledge of practical nursing procedures.
  • GS‑06 Level: At least one year of qualifying experience equivalent to the GS‑05 level; technically proficient in initiating, performing, and completing assigned duties; able to carry out patient care using judgment and accurate documentation.
  • Special responsibilities at each grade include medication administration, patient assessment, emergency response, equipment management, patient and family education, teamwork, and staff pre‑orientation as appropriate.
Duties
  • Collaborate in an interdisciplinary coordinated care delivery model.
  • Manage patient and clinic flow to ensure clinic efficiency.
  • Maintain effective and efficient communication with patients, the interdisciplinary team, VA medical centers, and other agencies.
  • Perform duties specific to grade level, including checking vitals, processing reminders, reviewing alerts, administering medication, attending huddles and meetings, setting priorities, and adjusting workflow to meet team and patient needs.
  • Assist in plan‑of‑care reinforcement and self‑help solutions; enter information into the electronic record.
  • Monitor pre‑appointment requirements to assure readiness for patient visits or procedures.
  • Participate and provide input in problem‑solving on operational issues or procedures in team meetings.
  • Follow up on team huddles, share information, and collaborate with the medical team to ensure continuity of care.
  • Evaluate patient information and clinic schedule lists to determine appropriate patient care actions.
  • Provide patient and family education regarding disease conditions, procedures, tests, and health maintenance.
  • Use the Six Rights of Medication Administration, including two patient identifiers.
  • Respond to medical emergencies and nursing alerts; seek assistance of an RN or MD/DO as needed.
  • Document, recognize, and report patient responses to medications and treatments.
  • Perform support duties for complex diagnostic tests and specialized procedures; assist in preparation, execution, and monitoring.
