Under general supervision, provide basic patient care to all patients, including those in isolation. Tasks include:

• Observation of patients with suicidal ideation, behavioral health issues, or other safety concerns; report significant changes to the primary Registered Nurse (RN).
• Documentation and reporting of all information within the scope of knowledge in the electronic health record (EHR).
• Conduct regular environmental assessments to ensure the environment is free of objects, equipment, or medications that could be used for self‑harm.
• Provide basic care (hygiene, activities of daily living, bed baths, showers, grooming, dressing, toileting, monitoring body functions, preventative skin care, incontinence care, feeding, oral hygiene).
• Assist nursing staff with mobility using safe transfer techniques, patient transport, range of motion, ambulation, restraints, and care of the patient’s environment.
• Obtain and record vital signs, temperature, height, weight, girth measurements, incentive spirometry, calorie counts, and other specific parameters as directed.
• Document findings on the appropriate EHR flow sheet and report to the RN.
• Perform patient treatments as directed, including application of heat or cold, simple non‑sterile dressings, ostomy care, post‑mortem care, and indwelling catheter care.
• Assist with oral suctioning, tracheostomy care, and long‑term feeding tube sites.
• Collect and transport specimens to the laboratory, obtain blood glucose levels, record fluid and food intake and output, remove urinary catheters, provide bowel care, place electrodes for ECGs, and manage telemetry and other monitoring devices.
• Recognize abnormal signs and symptoms (shortness of breath, rapid respiration, fever, chills, cough, chest or abdominal pain, cyanosis, drowsiness, excessive thirst or sweating); report changes to the RN.
• Initiate CPR and other basic life support or emergency measures as required; adhere to established safety, emergency, and disaster procedures.
