RN - Case Management

Day shift, 5 days a week, with hours varying between 07:00 and 17:00.

As a Registered Nurse Case Manager, you will be responsible for overseeing patient care and facilitating discharge planning in a Magnet-designated acute care hospital. You will work collaboratively with interdisciplinary teams to ensure that patients receive comprehensive care throughout their hospital stay and appropriate follow-up post-discharge.

• Coordinate patient care and discharge planning.
• Participate in multidisciplinary rounds and care coordination activities.
• Utilize electronic medical records (Epic) for documentation and communication.
• Ensure compliance with CMS and HIPAA regulations.
• Assess patient needs and evaluate care plans.

• Basic Life Support (BLS) certification required.
• Acute care nursing experience required.
• Experience with Epic electronic health record (EHR) system preferred.
• ACM certification is a plus.
