Under general direction, collects and analyzes data, identifies expected outcomes for each patient, develops a plan of care with interventions, and continually evaluates patient progress toward attainment of outcomes while collaborating with the specialty healthcare team and other disciplines.
Responsibilities- Use critical thinking and problem‑solving skills in a dynamic, systematic, collaborative approach to provide and coordinate health care services to a defined population.
- Apply advanced nursing specialty knowledge to assess, plan, implement, evaluate, and interact with patients.
- Evaluate options and services to meet individuals’ health needs through communication and available resources to promote quality, cost‑effective outcomes.
- Develop and maintain patient tracking and reporting programs to identify gaps in the health‑care delivery system.
- Contribute to ANTHC’s mission and vision by providing necessary coordination of care and participating in unit‑based councils and process‑improvement committees.
- Independently collect patient health data, analyze it to determine diagnoses, identify expected outcomes, develop and implement care plans, and evaluate progress.
- Actively participate in the systematic evaluation of nursing practice quality and effectiveness, researching and spearheading quality‑of‑care activities.
- Adhere to the Standards of Practice for Case Management and the ANA Nurses Code of Ethics; remain familiar with specialty standards if applicable.
- Consult on specialty issues with all involved disciplines and contribute to professional development within a shared‑governance nursing practice model.
- Make ethical decisions on behalf of patients, ensuring care delivery is non‑judgmental, nondiscriminatory, and respects patient diversity and autonomy.
- Collaborate with patients, families, and health‑care providers to establish overall goals and care plans, including decisions related to care delivery.
