Job Title
Duties include but are not limited to:
- Screens and coordinates care for unscheduled/urgent visits and clinical phone calls
- Nursing Clinic Visits
- Telephone visits
- Order/Consult placement
- Assessments
- Triage
- Care Plans
- Patient Education
- Clinical Reminders
- Patient intake
- Vaccinations
- Responds to secure messages
- Assists with Prevent. Care & stable chron. disease management
- Review of patient and Dashboards
- Coordinates care
- Chart reviews

Pay:
Competitive salary, regular salary increases, potential for performance awards
