Case Manager RN
Case Manager RN is needed to provide care coordination and support. Must possess a valid RN license. Experience in acute care is required. Epic experience is mandatory. Must be able to float within case management services as needed. Weekend work is required every other weekend or 4 weekend days in a 4-week schedule. No on-call responsibilities are mandated. The patient age groups served include neonates, adolescents, infants, adults, pediatrics, and geriatrics.
The daily census for the unit averages around 324 patients. Responsibilities include discharge planning and care coordination. Candidates must be familiar with CMS guidelines and HIPAA regulations. The individual will utilize evidence-based guidelines for determining medical necessity. Traveler must be willing to work during major holidays. Two references covering 1 year of work experience are required before starting. Effective communication skills are necessary for working with interdisciplinary teams.
Candidates will be expected to complete required training modules pre-start and annually.
