Case Manager RN
The Case Manager RN will be responsible for discharge planning. Must possess an AK RN license in hand for auto offer. Shift times may vary but will be between 07:00 and 17:00. Experience in an acute care hospital is required. Behavioral health experience does not apply. The position requires the ability to float within case manager services to cover gaps as needed.
Every other weekend or four weekend days in a four-week schedule are required. Candidates must be willing to work two out of three major holidays:
Thanksgiving, Christmas, and New Year. Hourly guarantees are based on 72 hours per two weeks, with the potential for call-off. Utilization of Epic software is essential. Candidates must demonstrate proficiency in care coordination and case management practices. Interdisciplinary collaboration with various support services is expected. The candidate should have experience with prior authorizations and medical necessity determinations based on evidence-based guidelines.
A strong understanding of HIPAA regulations is required. The role may involve education regarding discharge planning for patients across various age groups, including neonates, children, and geriatrics. This position does not allow for local candidates; candidates must reside at least 50 miles from the facility. References may be required during the compliance process prior to the start.
