We are seeking a Care Manager RN to provide comprehensive, patient-centered care management for at-risk patients in an acute care setting.
Responsibilities Include- Assessment and care planning
- Care coordination and resource management
- Discharge planning and documentation
- Ensuring regulatory compliance and advocating for patients
- Optimizing healthcare resource utilization across the continuum
- Associate's Degree in Nursing or higher
- Alaska RN license
- At least 2 years of acute care experience in Medical Surgical or inpatient settings
- Competency in Utilization Review
- Bachelor's Degree in Nursing or higher
- National certification in specialty area
- Experience in care management or utilization review, or completion of relevant training programs
- Competitive pay range of $45.04 - $79.13 per hour
- Full-time employment with multiple shifts available
