Success Manager

Position Summary

All Covenant House Alaska program staff work as advocates and allies for youth experiencing homelessness and human trafficking.

The Success Manager will build intentional and meaningful relationships with youth, engage them in a variety of programming, assist in providing desired referrals and work through their stages of change to drive toward long-term positive outcomes. The Success Manager is responsible for the ongoing engagement, assessment, and service planning with young people to meet their housing, health and wellness, employment and educational goals, as they move towards permanent housing.

The Success Manager will maintain a case load of 10-15 youth ages 13-24, experiencing homelessness and/or trafficking, who may have multiple barriers (e.g. serious mental illness, substance use disorders, chronic health conditions, frequent contact with law enforcement, etc.). Other functions include crisis intervention and de-escalation, documentation in multiple databases, facilitation of psycho-educational and support group activities, community engagement and networking with local service providers.

The Success Manager will work in partnership and collaboration with multiple agencies or programs and provide systems’ advocacy to help youth reach their identified goals.

Essential Functions

• Understand, practice, and promote the vision, mission, and five principles of Covenant House Alaska.
• Provide case management services in multiple settings and coordinate services with other agencies. Make appropriate referrals for services within CHA and to community agencies.
• Meet all minimum case management and documentation standards, including, but not limited to:
  • Perform intakes and discharge planning in line with CHA timeline.
  • Have weekly meetings with assigned youth to collaborate on their Success Plans and assist in goal attainment. Revise and update the success plan as necessary.
  • Keep youth success plans, case management efforts, and all other data points required while working with youth in our services in accordance with CHA Data Quality Standards.
  • Ensure each youth has completed a Coordinated Entry Assessment and verify that all information in the assessment is current, updating as necessary.
  • Complete data entry in all required data systems in accordance with CHA data quality standards, including ETO, AKHMIS, and grant-related databases.
  • Perform trafficking screenings (Quick Youth Indicators for Trafficking). Assist youth in safety planning around a variety of vulnerabilities; coordinate with the criminal justice and youth protection systems as required by law due to youth age (under 18), youth vulnerability (APS standards) or at the request of the youth (ages 18-24).
  • Complete Social Emotional Wellbeing, Resiliency, and other identified assessments.
• Participate in various collaborative internal and external meetings to advocate for and assist youth, including but not limited to Coordinated Entry meetings, child welfare team decision meetings (TDMs), multidisciplinary team meetings, case conferencing meetings, etc.
• Engage youth in appropriate educational, vocational, and life skill programming.
• Maintain a positive and professional working relationship with the general public, including other agencies and families of the youth we serve. Develop and maintain knowledge of and connection to community resources.
• Utilizing an Agency vehicle, transport clients to appointments and agencies to receive services, as needed.
• Assist in the operating needs and other elements of the program as needed, such as:
  • Ensuring the safety of youth seen by CHA at all times.
  • Answering phones, doors, and other needed program communication channels.
  • Participating in team, shift change, and staff meetings.
  • Assisting in packing and storing resident’s belongings, assisting residents with move in and move out along with preparing rooms for new residents.
  • Inspecting and assisting in maintaining rooms and common areas for cleanliness, maintenance, and safety issues.
  • Provide crisis de-escalation and intervention as necessary.
  • Filling in at various service stations as requested such as at the front desk, residential section, and the Community Center.
• Maintain a high level of ethical standards.
• Demonstrate an enthusiastic commitment to the job and the Agency.
• Maintain consistent communication with leadership and success team members.
• Perform other duties as assigned.

Qualifications

Education

High school diploma required. Bachelor’s degree in human services, social work, psychology, or a related field is highly preferred. Experience can substitute for post-secondary experience on a year-for-year basis.

Experience working with adolescent or disadvantaged populations preferred; life, volunteer, and internship experience will be considered. Lived experience considered an asset. Experience providing case management or closely related service provision is required.
