Aveanna Healthcare is hiring a Hospice Nurse Practitioner in Alabama to perform face-to-face encounters for hospice eligibility and recertifications. You will document findings, coordinate with the medical director and RN Case Manager, and participate in interdisciplinary team activities.
The role requires active nursing licensure as a Nurse Practitioner, the ability to travel, and strong clinical assessment skills in a home care setting. Part-time visits are scheduled Monday through Friday.
