- Flexible Hours – 5 day work week (40 hours, Monday thru Friday) NO Weekends Typically 8 am‑5 pm

- Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
- Conduct routine site visits, checking vitals, performing assessments, counseling medication administration, and documenting progress
- Act as a health/medical resource for staff, training staff in the medical arena
- Conduct audits and reviews of medications
- Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
- Full‑time Monday‑Friday with occasional travel required; flexible hours
- Participate in an on‑call rotation for triage of medical concerns for 1 week per month
- Current Licensed Practical Nurse (LPN) license in the State of Indiana required
- 1–2 years experience working with individuals with disabilities or other long‑term care is preferred
- Must be skilled, flexible, self‑motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Ability to work independently and efficiently
- Organizational and computer skills
- Acceptable background check & driving record check
