Home Health Registered Nurse (RN) – Compassus at Ascension at Home
Join our team to support patient recovery, enable home care, and empower patients and families with education and disease management.
Responsibilities- Support patient recovery and ability to stay at home
- Empower patients and families to reach healthcare goals by educating them on disease management
- Case manage and provide clinical oversight to interdisciplinary team
- RN license in the state of practice
- Minimum two years experience as a registered nurse, home health experience preferred
- Current driver’s license
- Current CPR and BCLS certification
