Hospice RN On Call Weekends

• Full‑time Hospice RN on‑call
• Friday 5:00 pm–11:00 pm;
  Saturday/Sunday 8:00 am–11:00 pm
• Territory:
  Respond to on‑call needs within an hour
• Perform physical and psychosocial assessment on all hospice patients
• Assess and re‑evaluate pain; apply appropriate pain‑management techniques and educate patient and family
• Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families
• Assess patient’s condition, initiate plan of care, re‑evaluate and update as needed
• Obtain necessary physician orders and collaborate with physician as patient’s needs dictate
• Initiate case conferences to discuss multidisciplinary team responsibilities, pain management, and bereavement counseling

• One year hospice experience required
• One year RN experience required
• Licensed Registered Nurse in the State of Indiana
• Current driver’s license
• Reliable transportation and insurance
