Join a senior-focused primary care team as a Nurse Practitioner, providing comprehensive outpatient care in a collaborative, patient-centered environment.
Responsibilities Include- Reporting to the Associate Medical Director and working onsite
- Diagnosing and managing moderate conditions
- Maintaining accurate documentation and medical records
- Coordinating referrals and collaborating with interdisciplinary teams for holistic patient care
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Current national certification (AANP or ANCC)
- Active, unrestricted NP license in SC
- DEA license and Medicare/Medicaid credentials or ability to obtain
- 2+ years of NP experience, preferably in primary care or Medicare Advantage settings with geriatric patients
- Experience with Athena EMR and familiarity with Medicare guidelines and HEDIS quality indicators
- Outpatient clinical setting with a standard schedule
