Duties & Responsibilities- Creates a process for communicating and collaborating with members of the inter-disciplinary team involved in the care of patients.
- Participates with the unit team in daily rounds with the patients and reviews assessments, sets goals, and prioritizes assignments.
- Screens patients admitted and assesses those patients at high risk for post discharge needs.
- Serves as a liaison for patients/families, nursing staff, and physicians.
- They are the vital link for community, health, and social agencies that have previously provided services for the patient or will be providing services post discharge.
- Provides the Medicare Important Message to patients prior to discharge as appropriate. Educates the patient and family on the discharge planning process and facilitates patient flow through the healthcare continuum.
- Arranging services in other medical facilities, other specialized institutions, or community agencies.
- Assists the patient and/or family in applying for financial assistance, services, or counseling to the appropriate community agency or other source of help.
- Validates payer coverage and assures appropriate agencies are utilized.
- Maintains expertise of available resources and federal and state medical assistance programs.
- Screens for abuse, neglect, or exploitation and refer appropriately to state or community agencies in accordance with state and federal regulations.
- Collaborate with team members to identify and validate avoidable days.
- Maintains professional integrity and confidentiality of information in accordance with AnMed policies and procedures, state and federal requirements, and applicable laws.
- Adapts quickly to changes in criteria and/or focus and fulfills duties responsibly with little supervision.
- Able to work with variable patient populations.
- Stays abreast of departmental and organizational changes.
- Recommends process improvements of policies and procedures as necessary.
- Performs other duties as assigned.
- Bachelor’s degree in social sciences or a four-year degree with related experience.
- Knowledgeable regarding community resources.
- Works in collaborative team approach.
- Two years of hospital discharge planning.
- One to two years of experience in social services or patient care experience.
