Weekend Clinical Liaison

As the Weekend Admissions Liaison, you will serve as the primary point of contact for weekend admissions, ensuring a seamless and supportive experience for patients and their families. This role requires strong clinical insight and regulatory knowledge to support safe and compliant admissions.

• Manage weekend inquiries and guide families through the admissions process
• Collaborate with nursing, rehabilitation, and administrative teams to confirm clinical eligibility
• Complete admission paperwork and collect all required documentation
• Ensure PASRR screenings are completed accurately and in a timely manner prior to admission
• Maintain clear and consistent communication with hospitals, families, and facility staff
• Represent the Vantage Care with professionalism, compassion, and positive energy

• Must be a licensed professional: RN, LPN, or Licensed Social Worker
• Experience in admissions within a healthcare or long-term care setting preferred
• Demonstrated knowledge of PASRR requirements and ability to complete screenings independently
• Strong communication skills with a customer-service-focused approach
• Highly organized with the ability to multitask and manage busy weekends
• Professional, approachable, and calm under pressure
