Physical Therapist - Hybrid SNF/Home Care
As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home (TYTH) Program. This position focuses on home health care in the Ann Arbor and surrounding areas, with a steady referral source from multiple skilled nursing facilities. You will supervise PTAs and have the opportunity to pick up additional time at nearby skilled nursing facilities.

Home Health services are provided in the vicinity around Ann Arbor.
ScheduleFull‑time, Monday‑Friday or Tuesday‑Saturday (Tuesday‑Saturday schedule will receive premium shift pay).
Required Qualifications- Successful completion of an accredited Physical Therapist program.
- Licensure as required by the State of Michigan.
- Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions.
- Initiative and flexibility in the work environment.
- Ability to respond effectively to the needs of patients and their families.
- Physical ability to bend, position, transfer, and lift patients weighing from 80 pounds to over 300 pounds without restriction, utilizing clinically recommended lifting devices.
