Responsibilities:
  • Providing care according to the patient’s plan of care, be able to fully implement the five phases of the nursing process (assessment, diagnosis, planning, implementation, and evaluation) and observe professional standards of care in the home care setting.
  • Basic home care/personal care skills (ie, ADL’s, nutrition monitoring, range of motion)
  • Clinically Competent in various areas of skilled needs (ie, tube feeding, trach care, respiratory care, etc)
  • Assure proper maintenance of clinical records in compliance with local, state, and federal laws.
Requirements:
  • Current RN License in good standing
  • Certified CPR or BLS
  • Preferred 2-5 years’ experience caring for peds is preferred but not necessary
  • Must be able to implement all five phases of nursing process (assessment, diagnosis, planning, implementation, and evaluation, Trach care)
  • Must observe professional standards of care in the home setting under the supervision of the Director of Nursing
  • Licensed driver with insured and reliable automobile
  • Empathy for the needs of the client
  • Ability to read, write, speak and understand English and communicate effectively
  • Ensures confidentiality and security of the client’s medical information.
  • Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift to 50 pounds
