Job Summary

BMC2 is a collaborative consortium dedicated to improving the quality of care and outcomes in patients who undergo cardiovascular procedures across the State of Michigan. BMC2 is developing a new heart failure registry, which will take a population health-based approach to measuring outcomes and improving quality of care for patients with heart failure. The new registry will bring together diverse stakeholders including patients, specialists, primary care physicians, and pharmacists.

Support for BMC2 is provided by Blue Cross and Blue Shield of Michigan and Blue Care Network as part of the BCBSM Value Partnerships Program.

BMC2 is seeking an experienced Project Senior Manager to support the design, coordination, and implementation of this new initiative entitled BMC2-HF. This role will report to the BMC2 Program Manager and will work collaboratively with core program leadership, external partners, and current members of the BMC2 team. This role will help shape the strategic direction of the initiative while supporting day-to-day operations as the program matures.

Responsibilities
* Program Development

* Support the development and refinement of BMC2?s heart failure implementation strategy, including but not limited to data strategy, initiative goals, priorities, and program scope.

* Identify opportunities for innovation and collaboration related to heart failure quality improvement.

* Support the Program Manager in the recruitment and onboarding of new HF Coordinating Center staff.

* Identify and build relationships with healthcare partners statewide to support registry expansion

* Strategically identify measures that will allow for the development of statewide interventions aimed at improving heart failure quality of care

Project Management & Execution

* Lead the planning and scoping of multiple goals and deliverables. Prepare detailed project planning documents and serve as the primary point of contact for project activities, timelines, milestones, and deliverables.

* Lead the development and implementation of related tools, workflow documentation, and other related support resources

* Track project milestones, deliverables, and risks, escalating issues as needed to ensure timely and successful outcomes.

Communication & Evaluation

* Build and execute effective communication plans to ensure our partners receive timely updates to remain engaged & mission driven.

* Partner with the BMC2 Communication Specialist to develop internal and external communications related to the initiative (e.g., social media, blogs, monthly newsletters and annual reports).

* Significantly contribute to deliverables to our funder, including annual and bi-annual reports, scope of work, and other deliverables as needed.

* Utilize available data, analytical tools, benchmarking, and metrics to inform decision making as well as determine impact before, during, and after project completion.

Continuous Improvement & Growth

* Remain adaptable as the initiative evolves, taking on new responsibilities as priorities emerge.

* Identify opportunities to align the heart failure initiative with other BMC2 programs, partnering CQIs, and state and national efforts.

* Demonstrate growth mindset to actively seek opportunities to learn, adapt, and apply new skills and knowledge to enhance team and personal performance.
