Position Description

The Registered Nurse (RN) is responsible for providing general nursing care to our patients through collaboration with Physicians and other support staff of the Center. The Registered Nurse will promote and strive to restore a patient's health through education, advocacy, and quality delivery of care.

This role requires exceptional teamwork, adaptability, and strong communication skills to address the dynamic and evolving needs of patients. Reporting to the Health Center Practice Manager, the RN is integral to aligning care delivery with Patient‑Centred Medical Home (PCMH) and Accountable Care Organization (ACO) standards, ensuring high‑quality, patient‑focused outcomes through coordinated efforts and innovative problem‑solving.

Qualifications
  • Current RN licensure in the state of Michigan
  • Minimum education requirement:
    Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
  • At least 3‑5 years of relevant experience in primary care, chronic care management, disease management, home health, or ambulatory care preferred
  • Experience working for a community health center or Federally Qualified Health Center preferred
  • Strong relationship‑building skills with patients, staff, and providers
  • Proficient in using EHR systems and Microsoft Office Suite (Excel, Word, PowerPoint)
  • Excellent written and oral communication skills
  • Expertise in motivational interviewing and other behavioral health strategies to promote self‑management
  • Knowledge of physical and behavioral health medications and screening tools
  • Ability to analyze and utilize data to drive care management decisions
  • Strong organizational and project management skills
  • Participation in ongoing professional development and competency‑based evaluations after hire are mandatory
