Role: Registered Nurse (RN) - Case Manager
Comp: $85,000-$105,000 approximate yearly pay
$64.10 per point
Mileage in addition to productivity comp
Schedule: Full-time Monday–Friday + some weekends (as needed)

The mission of this roleDeliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
ObjectivesCarry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review

Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
Direct, high-acuity patient care, with a focus on wound care
Building and managing the plan of care with physicians;
supervising LPNs and HHAs on your cases.Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
Dedicated clinician: home health experience strongly preferred
High ownership: you treat your caseload like your own panel and your pod like your own business.

Active Michigan RN license
Reliable transportation, valid driver's license, auto insurance
Home health / OASIS experience strongly preferred
Full-time target: ~30 productivity points/week
; clinicians earn more for exceeding it.Guaranteed floor: ~21 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor.
$1k sign-on bonus paid after first 90 days
$1k bonus for referrals (we’re growing)!
