Position: Occupational Therapist (OT)

Role:
Occupational Therapist (OT)

Schedule:

Full-time, Monday–Friday with occasional weekends as needed.

Location:

Field Based in Detroit, Michigan.

Compensation: $100,000 – $130,000 per year, plus potential productivity bonus ($75.00 per point). Guaranteed floor of ~24 points/week for first 90 days; target ~28 points/week.

Responsibilities
  • Carry a full, high‑quality caseload and ramp to ~28 productivity points per week.
  • Own clients’ outcomes – drive low avoidable hospitalization rates and strong patient experience scores.
  • Participate in case conferences and team huddles to improve quality.
  • Serve as a referral‑source ambassador, ensuring discharge planners, assisted living facilities, and physicians refer patients to the organization.
  • Set a standard for charting, cadence, and judgment that other clinicians emulate.
  • Complete in‑home OT evaluations and deliver skilled OT visits.
  • Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
  • Recommend and train on adaptive equipment and home modifications as needed.
  • Coordinate with nursing and the broader care team when condition or needs change.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) – defensible, efficient, and consistent.
Qualifications
  • Active Michigan OT license in good standing.
  • Strong clinical judgment and comfort working independently in the field.
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
  • Strong communication and documentation skills.
  • Reliable follow‑through on orders, frequencies, and patient goals.
  • Valid driver’s license, auto insurance, reliable transportation, and comfort with daily travel across assigned territory.
  • Ability to carry/transport basic therapy equipment as needed.
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education).
  • Prior home health experience (nice‑to‑have).
  • Homecare Homebase (HCHB) experience or similar home health EMR (nice‑to‑have).
  • Experience with high‑signal OT populations (neuro, CVA, fall risk, dementia/caregiver training) (nice‑to‑have).
  • Strong organization and time‑management skills (nice‑to‑have).
