In-Home Health - Nurse Practitioner or Physician Assistant Colorado Springs

Full-time Clinician (Nurse Practitioner or Physician Assistant)

Signify Health, part of CVS Health®, is seeking a full‑time clinician to provide in‑home health evaluations, engaging with patients in the comfort of their own homes and helping bridge gaps in care.

• Conduct approximately 45‑minute in‑home visits, including medication and medical history review, a physical evaluation, and additional diagnostic tests as ordered by the health plan (e.g., diabetic eye exams, spirometry).
• Use an iPad with an intuitive clinical workflow for each evaluation.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).
• Join a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Assist in visiting millions of people nationwide with the goal of connecting them to the right care for their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the specific role requirements.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as required.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state license(s) (preferred).
• Ability to cover the primary location:
  Colorado Springs.
• Clinical experience in nursing or physician assistant practice.

• Experience with in‑home care or telehealth services.

Full‑time, 40 hours per week.

$87,035.00 – $ (base hourly rate or base annual full‑time salary; actual offer depends on experience, education, geography and other factors).
