Position: Clinical Care Coordinator: Ambulatory Care Services (C & W Mott Hospital IPOP Unit)

Responsibilities

  • Coordinator will be expected to cross train to all areas of IPOP
  • Provide quality care to patients in IPOP
  • Navigate and triage patient plan of care in IPOP
  • Communication and coordination of care with patient and family within the IPOP experience
  • Coordinate with research team to facilitate patient care
  • Work as a nurse member of a collaborative multidisciplinary team; both in-patient, IPOP, and out-patient
  • Provide clinical direction and support for the Cell and Gene Therapy team through consultation with nursing and other team members
  • Provide continuity and coordination for patient population in IPOP
  • Coordinate and participate in the development and implementation of patient and nursing education programs and materials relating to IPOP
  • Active part of the research team
  • Coordination of admissions to hospital, IPOP and OTEC

  • Willingness to adapt to different populations as patient demands
  • Demonstrated accountability and dependability for completion of work, mandatories, and goal setting within established timelines
  • Excellent interpersonal (including phone) skills with physicians, peers, and patients as demonstrated by previous employment, references, and evaluations are a must
  • Demonstrated ability to organize and delegate effectively and efficiently
  • Ability to work collaboratively with physicians and other team members
Qualifications
  • Current nursing license to practice nursing in the State of Michigan
  • Bachelor's degree in nursing or associate's degree/diploma in Nursing along with a master's degree in nursing
  • Three (3) years of Cellular Therapy and Bone Marrow Transplant RN experience
  • Ambulatory care or in-patient experience within the last five (5) years
  • Registered nurse coordinator experience; preferably one (1) year in the past three (3) years
  • Lead nurse or charge nurse experience; prefer six (6) months in past three (3) years
  • Proven leadership ability as evidenced by previous employment and references
  • Demonstrated ability to set goals, promote teamwork and problem solve as demonstrated by previous performance evaluations
  • Experience with MICHART, EPIC, Cisco Finesse, WORD, and Excel
  • Certification from a nationally recognized organization
  • MSN degree
Hours & Shift
  • 40 hours per week (1.0 FTE)
  • Day Shift, Monday–Friday. Flexible hours to meet program needs; may require weekends and holidays
Location
  • This position is located in the IPOP unit, CW Mott Hospital; varies

Note:

All new employees will be expected to float to designated unit(s) in times of low census.

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
