Job Summary
A Division of Nephrology seeks an Advanced Practice Provider for its Outpatient Hemodialysis Unit – Ann Arbor Dialysis Center. The position is for 0.5 FTE, 20 hours per week, requiring either a Nurse Practitioner or Physician Assistant with core responsibilities outlined below. The role reports to the facility medical director and attending nephrologists.
Responsibilities- Work under the supervision of the facility medical director and attending nephrologists to provide patient care in the outpatient hemodialysis unit.
- Interview new patients to obtain and document a detailed patient history and perform focused physical examinations.
- Collaborate with nephrologists to provide face‑to‑face interval assessment, evaluate the need for changes in the dialysis prescription, and assist in writing weekly progress notes.
- Work cooperatively with other members of the dialysis unit team and participate in regularly scheduled Plans of Care.
- Assess and treat acute medical problems identified through ongoing interval assessment, order appropriate tests and procedures, and/or make appropriate referrals.
- Coordinate care with other medical providers both inside and outside the University of Michigan.
- Monitor labs and assist in the development of patient‑specific, facility‑based disease management protocols.
- Advise patients of monthly lab results and provide patient education.
- Provide professional guidance to facility personnel in the implementation and maintenance of facility‑based protocols such as anemia management, vascular access surveillance, and bone disease management.
- Participate in program QA/CQI efforts.
- Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
- Current specialty certification to practice as an Adult‑Gerontology Acute Care Nurse Practitioner, Family Nurse Practitioner, or Adult Nurse Practitioner in Michigan and a graduate of an accredited nurse practitioner program.
- Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
- Current DEA registration required.
- Current AHA Approved BLS, ACLS certification preferred and can be obtained within 30 days of hire.
- Current license to practice as a Physician Assistant in the State of Michigan.
- State of Michigan Controlled Substance license required.
- Completion of a Physician Assistant program accredited by ARC‑PA, CAHEA, or CAAHEP.
- Certification by the National Commission on Certification for Physician Assistants (NCCPA).
- Current DEA registration required.
- Current AHA Approved BLS, ACLS certification preferred and can be obtained within 30 days of hire.
Employment is contingent upon successful completion of the credentialing and privileging process. Applicants must have met all the required qualifications prior to the start date of employment.
Desired Qualifications- 2 years of recent experience (within the last 5 years) as an APP.
- Experience managing patients with chronic kidney disease on hemodialysis.
- Demonstrated ability to complete history and physical examinations; order appropriate diagnostic studies, synthesize information and make informed clinical decisions; develop appropriate care plans; and be responsive to patient and family preferences.
