Responsibilities
- Assess, plan and implement patient care; educate patients and families on disease processes, procedures and medications; and address various patient needs, including, but not limited to, IVs, PICC lines, lab draws, wound care, trachs, G/J tubes, Foley caths and medication administration.
- Build satisfying one-on-one, long-term relationships and witness positive outcomes as patients regain health and increase independence.

- Autonomy: use your knowledge, skills and judgment to independently provide the highest level of treatment, as well as work additional hours, as desired.

- Excellent Training: orientation is robust with lecturing, field time, shadowing and preceptor support, but training doesn’t stop there. - One year of acute care experience.
- Current RN state licensure.
- CPR certification.
- Ability to travel for in‑home patient visits.
- Exceptional time‑management skills.
- Excellent interpersonal and customer service skills.
- Home healthcare/case management experience preferred.
