Support outreach clinics for all organs in all locations. Accurate and efficient documentation of nursing care. Knowledge and appropriate use of internal and external resources. This RN position is essential in managing acute and chronic patient care for pre/post-transplant patients. Provide skilled observations, telephone triage and health assessments of patients including history, appropriate interventions, and referrals. Manage patients post procedural and/or for IV infusion therapy.

Foster continuity of care and facilitate transitions across outpatient and community providers on patient population. Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice.

Critical thinking skills through the nursing process, excellent problem-solving skills, and demonstrated ability to set priorities. Customer service, interpersonal communication skills, and the ability to build respectful relationships within and between units and individuals. Adaptability to changing needs and priorities based on unit, department, and institutional goals/objectives. Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings.

Proficiency with typing and electronic health record documentation. Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions. Willingness to adapt to different populations within Transplant Center after successful Orientation.

Current licensure to practice nursing in the State of Michigan. 3 (three) years of nursing experience as an RN, within the last 5 (five) years Ambulatory Renal Transplant Nursing experience within the last 5 (five) years Demonstrated competency in the care of immunocompromised patients Transplant Certification (CCTC/CCTN) within three years of hire.

1-2 years experience caring for abdominal transplant patients within the last 5 (five) years 2-5 experience in the Ambulatory Care setting within the last 5 (five) years. At least 2 (two) years experience in Operating Room, Procedural Area, or ICU setting Experience in the acute care of abdominal transplant patients 3 (three) years nursing experience in the inpatient setting. Knowledge of UMHS nursing policies and standards of practice.

Bachelor of Science Degree in Nursing.

• Evening Shift Differential-$3.00 / hour
• Night Shift Differential- $4.00 / hour
• Day Shift Weekend Differential- $2.90 / hour
• Evening Shift Weekend Differential- $5.90 / hour
• Night Shift Weekend Differential- $6.90 / hour
• Charge Nurse Differential- $1.00 / hour
