Job Overview

Signify Health, part of CVS Health®, seeks a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The role involves engaging patients in their homes, conducting medication and medical history reviews, physical evaluations, and, when ordered, additional diagnostic tests.

• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).

• Active, unrestricted NP or PA license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover primary locations:
  Iron Mountain (Dickinson, Iron, Menominee counties).
