Responsibilities*Accurate and efficient documentation of nursing care
Knowledge and appropriate use of internal and external resources.
This RN position is essential in managing acute and chronic patient care for post-transplant patients. Provide skilled observations, telephone triage and health assessments of patients including history, appropriate interventions, and referrals.
Foster continuity of care and facilitate transitions across outpatient and community providers on patient population.
Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice.
Potential to support outreach efforts.

- Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings.
- Proficiency with typing and electronic health record documentation.
- Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions.
- Willingness to adapt to different populations within Transplant Center after successful Orientation.
Current licensure to practice nursing in the State of Michigan. Three (3) years of nursing experience as an RN One to two years of experience caring for solid organ transplant patients as a RN Demonstrated competency in the care of immunocompromised patients Transplant Certification (CCTC/CCTN) within three years of hire.

Recent 3 years RN experience in the inpatient setting
2-5 experience in the Ambulatory Care setting as a RN
At least 2 years' experience in Operating Room, Procedural Area, or ICU setting as a RN
Experience in the acute care of transplant patients as a RN
Knowledge of UMHS nursing policies and standards of practice.
Bachelor of Science Degree in Nursing.
