Responsibilities

Provide the delivery of culturally sensitive, age-specific team-based care models in delivering nursing care across the continuum for Primary Care patients including, peds, adult medicine and geriatric patient populations. Perform phone triage, including assessment, screening, clinical advice, nurse visits (in person/ video visit) in-clinic hands on procedures, care coordination with internal and external partners and other duties assigned. Accurate and efficient documentation of nursing care.

Knowledge and appropriate use of internal and external resources. Provide functional supervision to assistive personnel; collaborate with colleagues, other disciplines, and health care/community services to promote continuity of care.

Expectations

Demonstrated critical thinking skills through use of the nursing process, excellent problem-solving skills, and demonstrated ability to set priorities. Demonstrated customer service, interpersonal communication skills, and the ability to build respectful relationships within and between units and individuals. Demonstrated accountability and dependability including a good attendance record with punctuality. Demonstrated adaptability to changing needs and priorities based on unit, department, and institutional goals/objectives.

Demonstrated knowledge and skills providing patient and family education and coaching related to chronic conditions including, but not limited to, diabetes, asthma, and coronary artery disease. Proficiency with typing and electronic health record documentation. Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions. Maintain competencies to care for multi-populations ie: pediatrics, OB/GYN, adult medicine as patient demands.

Orientation and training would be provided for new subspecialties within primary care based on expansion of division.

Required Qualifications

Current license to practice nursing in the State of Michigan (RN). Five (5) yrs of RN experience in last 7 years working with General Medicine and Geriatric patients. Two (2) yrs (out of the last 7 years) working in Post-Acute Care Services or Primary Care in a RN Care Navigator Role.

Desired Qualifications

BSN Certification in Ambulatory Nursing 3 years of Ambulatory Care Phone Triage experience
