Home Health Aide

The individuals served by this position may face many challenges including multi‑morbidity conditions, mental illness, substance abuse disorders, and/or chronic medical conditions alongside a history of chronic homelessness. Home Health Aide duties provide hands‑on support with these clients in their homes to ensure their well‑being and maintain housing. Provided services include apartment cleaning, laundry, skills coaching, and well‑being assessments.

Job Responsibilities
  • Working with Avalon clients throughout the community to support their housing permanency.
  • Cleaning and organizing tenants’ apartments as described in the tenant’s treatment plan or inspection results to assist tenants with maintaining housing.
  • Assisting with laundry chores, meal preparation and clean‑up, and shopping for food, hygienic products and home‑care products.
  • Teaching tenants proper cleaning techniques and incorporating cleaning habits into their routines.
  • Recognizing potential crisis or emergency situations, analyzing them accurately, developing strategies, and informing the supervisor.
  • Assisting the case‑management team in identifying potential problems and providing input to client program plans.
  • Cooperating with clinical, property‑management, maintenance, and other staff onsite, and referring tenants to additional services as necessary.
  • Monitoring clients’ well‑being and sharing observations with the team.
  • Assisting clients in attending appointments and providing transportation when needed.
  • Providing emotional support and feedback to clients, families, and friends.
  • Maintaining written reports on clients’ activities and progress, ensuring documentation is complete and accurate.
  • Participating in group planning and helping develop individual activity plans focused on clients’ special needs.
  • Providing lay counseling support and life‑skills training to clients.
  • Operating office equipment such as personal computers, photocopiers, facsimile machines, and printers.
  • Maintaining up‑to‑date knowledge of fire safety regulations, emergency procedures, and overall health and safety standards.
  • Performing other related duties as required.

Job Duties
  • Work with clients in their homes, community centers, and the larger community.
  • Assist tenants with conflict resolution among tenants and neighbors.
  • Attend and participate in staff meetings, retreats, and professional training sessions.
  • Facilitate group transportation as needed.
  • Cover appointments, transportation needs, and Residential Support Specialist shifts as required.
Required Qualifications
  • At least one year’s experience in human services, preferably with similar populations (homeless, mental illness, chronic health, or addiction disorders).

  • Basic understanding of homelessness and the characteristics of the homeless adult population; experience with mental illness and chemical dependency preferred.
