Position: CLIENT SERVICES MANAGER Y&F - UP

INTERNAL UNION APPLICANTS WILL BE GIVEN FIRST CONSIDERATION FOR UNION POSITIONS

Under the supervision of a qualified professional, the Client Service Manager (CSM) position provides services to children, youth, and their family. The youth may have a diagnosis of a severe emotional disturbance, autism spectrum disorder and/or a developmental disability. Trauma informed; strengths-based approaches are utilized for safety planning, advocacy, referrals, and parenting support. Under the direction of a supervisor, the CSM is responsible for updating assessments, developing goals, and linking children + families to resources and services that improve, maintain, or restore an enhanced quality of life.

Working within the team context assures that the Family Centered Process results in an integrated plan of service and facilitates the achievement of specific outcomes. In their role, the staff person is responsible for developing and maintaining the Family Centered Plan (FCP), coordinating and monitoring youth/family services, and directly assisting youth and families in achieving desired outcomes and documenting consumer progress according to WCCMH documentation standards.

EXAMPLES

OF DUTIES Core Functions within the context of the Family Centered Practice Assessment and Family Centered Planning
  • Completes comprehensive consumer needs assessments, including updated assessments based on changes in consumer needs or circumstances
  • Leads the development and maintenance of Individual Plan of Service (IPOS) using a family centered planning process for all assigned consumers, assuring that IPOS are compledted within mandated time frames
  • Assures that the IPOS is family focused
  • Assures that all outcomes are specific with regard to amount, scope, and duration of service
  • Completes progress reviews at intervals defined by most current departmental policy
Foster resiliency, skill development, and improved quality of life
  • Develops supportive relationship with youth and families to insure that the Family Centered Planning process is consumer/family driven
  • Coordinates and collaborates with the youth /family network of care, including schools, primary healthcare, family and natural supports
  • Monitors and coordinates service delivery to consumer/families as specified in the IPOS
  • Works with youth/families to promote the outcomes specified in the IPOS
  • Directly supports youth and families in resolving crises, coordinating additional services or supports where needed
  • Supports youth and families in the development of crisis plans, which are incorporated in the IPOS
  • Assists youth/family in developing resiliency and skills in coping/resolving issues
  • Identify and link to community resources that support integration to community and natural supports
  • Provide supportive counseling within the context of other ongoing services and refer to more intensive level of services when clinically indicated
Resources/Collaboration and Advocacy
  • Assist families in identifying and linking to needed services and entitlement available in the community
  • Assists in systems advocacy to secure needed services and resources either internal to the CMH system of services or external community system service providers (eg. Schools, DHS, etc.). Also links to family support networks within the community
  • Provides support and education regarding mental illness and stigma regarding mental illness and/or developmental disabilities
  • Inform youth/families regarding their rights and navigation the mental health system
Record Keeping
  • Documents consumer activity in relation to the outcomes specified in the IPOS
  • Maintains complete and detailed records consistent with agency standards, procedures and requirements of third party funding and accreditation agencies
  • Keeps timely and accurate clinical records in the electronic medical record system, including scheduled appointments, IPOS, progress reviews, assessment updates, progress notes, demographics and all other documentation as identified by the department or its funders
  • Obtain all necessary parental consents and releases of information necessary to provide services

  • Assures professional, ethical behavior and relationships
  • Observes all requirements of law and regulation with respect to Recipient Rights and confidentiality of health care
Medication Support
  • Provides oversight/assistance to consumer's requiring observance of medication adherence according to agency policy and after successful completion of medication training requirements

Other duties may be assigned by the Health Services Supervisor or Program Administrator dependent on program needs.
